The GP practices in our Primary Care Network are here to support you with your health and wellbeing. One of the ways we’re now able to do this is by inviting our patients and their carers to webinars that may be relevant to their condition.
On Friday 4th December we’ll be supporting Alzheimer’s Society with their fundraising and awareness raising Elf Day event. To mark the occasion our Social Prescribing team will be hosting a webinar where they will signpost you to the support available from Alzheimer’s Society and other local services for people with a dementia diagnosis, their carers and families.
